First, try cleaning the disc with a soft cloth and make sure there's no dust or scratches. If that doesn't work, rebuild the PS4 database by starting in safe mode and selecting the 'Rebuild Database' option. Sometimes, a quick system restart can do the trick too!

My go-to is always a safe mode database rebuild! Boot your PS4 into safe mode, choose 'Rebuild Database,' and it works wonders.
