Go to Xbox settings > Volume & audio output > Party chat output, and set it to 'Headset'. Easy as pie!

I had this issue too! Try lowering your microphone sensitivity in the Xbox settings. It worked wonders for me!

First, make sure your controller and headset are properly connected. Sometimes just unplugging and replugging them works like a charm!
