First, try lowering your headset volume. Sometimes feedback happens because your mic picks up the sound from your speakers!
Check if someone else's mic is causing the echo. Ask your friends to mute their mics one by one to isolate the issue.
Sometimes, a firmware update for your controller can fix the issue. Go to Settings > Devices & accessories, then select your controller and update it.
