OMG this drove me nuts! Make sure your mic is not near your speakers and reduce your volume. Move your mic away from your mouth a bit too.

First, try lowering your headset volume and the mic sensitivity. If that doesn't help, check if anyone else in the party is causing the echo by muting them one by one.

Used to have this problem all the time. Try updating the firmware on your controller and headset. Solved my issue instantly.

Quick fix: Turn off voice feedback in the settings. No more echo!
