First, give the disc a good clean! Sometimes a simple wipe with a microfiber cloth does wonders. Make sure it's clean and free of smudges or scratches.

I had this issue, and resetting the PS4 by completely turning it off (not rest mode) and unplugging it for a few minutes actually helped!
