First, try restarting your PS4. If that doesn't work, you'll probably need to rebuild the database. Boot into Safe Mode and select 'Rebuild Database' from the menu.

Holding the power button for about 7 seconds until you hear a second beep can also help. This can sometimes fix Safe Mode issues by forcing a complete shutdown and restart.

My PS4 got stuck in Safe Mode once. I fixed it by updating the system software using a USB drive. You just need to download the update file from the PlayStation website, stick it on a USB, and then use the 'Update System Software' option in Safe Mode.

Honestly, sometimes you just need to reinitialize your PS4. It will wipe your data, so back up your saves first, but it will give your system a fresh start and often solves Safe Mode problems.
