Check your HDMI cables and ensure they are securely connected. Sometimes, wonky connections can cause safe mode issues.
If restarting doesn't work, you can also try rebuilding the database from the Safe Mode menu. Just choose option 5 in the Safe Mode options.
From personal experience, I had to initialize my PS4 to fix Safe Mode problems. Choose option 6, but remember to back up your data first!
Make sure your PS4 is not overheating. Give it some breathing room and clean any dust off the vents.
