Getting a Bandage in Terraria is all about patience and a bit of luck. Focus on defeating Angler Fish or Werewolves as they are the ones who drop Bandages. Angler Fish can be found in Underground and Cavern layers of water, while Werewolves appear during a Full Moon on the surface. Keep at it, and you'll snag a Bandage eventually!

From my experience, spelunking for Angler Fish is the way to go for Bandages. They seem to drop more frequently from them than from Werewolves, at least for me. Plus, underground exploration is always full of surprises!
