Easy peasy! Just go into build mode, click on 'Furniture', then 'Doors', and plop it where you want. Voila!
From my experience, head to the build menu, choose the doors section under furniture, and place it at your restaurant's entrance.
To get a door in Restaurant Tycoon 2, go to the furniture section in the build mode, then select 'Doors' and pick the one you like!
