I've had luck by manually ejecting the disk. Turn off the PS4, unplug it, then use a screwdriver to rotate the manual eject screw (check the manual for its location). Worked like a charm for me!
Just go to Safe Mode by holding the power button for 7 seconds when the console is off. Then select 'Eject Disk' from the menu.
Hold the PS4 vertically and give it a gentle shake. Sometimes gravity works better than a screwdriver.
If none of that works, you might have to disassemble the console. Make sure you're comfortable doing it or take it to a technician.
