After you complete a Neutral route without killing anyone, go back and make sure you’ve befriended Papyrus, Undyne, and Alphys. This will trigger the special extra content needed for the True Pacifist ending. Just remember, no killing!

You'll need a neutral run first where nobody dies. Then, befriend Papyrus, Undyne, and Alphys. After that, continue the game and you'll get the True Pacifist ending.

To get the True Pacifist ending in Undertale, you need to avoid killing anyone and also be friends with Undyne, Papyrus, and Alphys. Keep sparing enemies, and don't forget to hang out with Papyrus and Undyne after befriending them. Once you meet these criteria, you'll trigger a special sequence after finishing the Neutral ending.
