Getting your hands on a Bloody Tear in Terraria is pretty straightforward but requires some patience and a bit of luck. You'll need to defeat enemies during a Blood Moon event, which randomly occurs at night. Keep in mind, the chance of getting a Bloody Tear is not 100% after every Blood Moon, so you might have to endure multiple Blood Moons to snag one. It's dropped by enemies during this event, so the more enemies you defeat, the higher your chances of obtaining it. Happy hunting!

Short answer: Just fight a lot during a Blood Moon. The Bloody Tear will eventually drop from an enemy. Good luck!

I actually got a Bloody Tear on my first Blood Moon by accident. Was just trying to survive the night and bam, it dropped. So, keep fighting and it might just happen!
