First, unlock Blue Baby (???). Then, defeat Isaac in the Cathedral with Blue Baby to get the D6 for Isaac. Easy peasy!
It's simple: Beat Isaac with Blue Baby (also known as ???) in the Cathedral. After that, you'll have the D6 whenever you play as Isaac.
Just beat Mom's Heart 10 times to unlock It Lives, then beat It Lives and reach the Cathedral. Defeat ??? there and boom, D6 is unlocked for Isaac.
You need to defeat Isaac as Blue Baby in the Cathedral! Once you do that, every new run with Isaac will start with the D6 as an active item. Good luck!
