First, unlock Blue Baby (???). Then, defeat Isaac in the Cathedral with Blue Baby to get the D6 for Isaac. Easy peasy!

Just beat Mom's Heart 10 times to unlock It Lives, then beat It Lives and reach the Cathedral. Defeat ??? there and boom, D6 is unlocked for Isaac.
