Defender Medals in Terraria come from one main source: the Old One's Army event. To trigger this event, you'll need to talk to the Tavernkeep, who appears after you defeat the Eater of Worlds or the Brain of Cthulhu. He'll sell you an Eternia Crystal Stand and Eternia Crystals. Place the stand, then use the crystal to start the event. Survive the waves of enemies, and you'll earn those shiny Defender Medals as a reward.
