You start with your city hall design based on the civilization you choose. To change it, use the ‘Civilization Change’ item you can buy with gems.
I've done this a bunch! Just make sure you've saved up those gems to get a Civilization Change token, then pick a new civilization to get the different city hall design.
You simply select a new civilization from your settings after using a Civ Change token. Easy peasy!
