Forget fiddling in the game, go straight to the Fallout4Prefs.ini file in your Documents/My Games/Fallout4 folder. Open that bad boy with Notepad, and find the line that says bFull Screen=0 and change it to bFull Screen=1. Also, make sure iSize H and iSize W match your monitor's resolution. Save it, and voila, fullscreen Fallout 4.
Alt+Enter is your quick fix friend. If you're in-game and it's not in fullscreen, just press Alt+Enter. That should flip it from windowed to fullscreen mode without you having to navigate through the menus. Works like a charm.
