To get into Safe Mode on your PS4, turn off the console completely. Then, press and hold the power button until you hear two beeps. Just hold the power button for 7 seconds from shutdown until you hear the second beep. Connect your controller via USB and hit the PS button.
From experience, I've found that sometimes you have to really make sure the PS4 is fully off鈥攏o rest mode! Then hold that power button till you hear beep number two.
