To get the True Pacifist ending, you got to first beat the game in Neutral mode. After that, befriend Papyrus, Undyne, and Alphys. Then go back to Alphys' lab for some extra content and follow through to the end. No killing allowed!

True Pacifist is the way! You just need to be friends with everyone and spare all the monsters. Don't harm a soul!

I got the real ending by making sure I spared every single monster and did all the side quests to befriend characters. Took a bit of time but totally worth it!
