You have to be on the Genocide route to get the Real Knife. It's in New Home, in Asgore's house, near the end of the run.
Real Knife? Take the Genocide path and you'll find it in New Home. It's in the room with the heart-shaped locket.
Honestly, it's only useful if you're going full Genocide. It's in New Home.
