First-hand experience here: just grab the course editor, find the '?' block in the menu, and place it wherever you like. I love hiding power-ups in them!
Easy peasy! Just go into the course editor, select the '?' block from the menu, and place it where you want. Voila, secret block!
To get secret blocks in Super Mario Maker 3DS, just tap the block icon in the course editor and look for the '?' block! You can hide all sorts of goodies in there.
