To unlock The Forgotten, you gotta beat the first floor's boss within a minute. Then you'll hear Mom call out to you, and a shovel piece will appear in the starting room. Use the shovel to dig up The Forgotten's grave in the Dark Room!

I managed to unlock The Forgotten after countless tries. Beat the first floor's boss in under a minute, then go back to the starting room to find the Broken Shovel. You'll need to complete Boss Rush to get the second part of the shovel. After that, use it in the Dark Room to dig up The Forgotten!
Speed is key! Take down the first boss within a minute and grab that shovel. Good luck!
