I did it by sparing every monster and then spending extra time with Papyrus, Undyne, and Alphys after my first neutral run. Felt great!
Hey! Just be nice to everyone and don't kill anyone! After completing the Neutral route, you have to go hang out with Papyrus and Undyne, and then go on a date with Alphys. Finally, head back to the True Lab and you've got it!
To get the True Pacifist ending in Undertale, you need to complete the game without killing any monsters, then go back and complete the pacifist side quests. Have fun making friends instead of enemies!
Avoid the Genocide route if you want the True Pacifist ending. Just sayin'.
