I usually give them a medical item from my inventory. Works like a charm every time.
You can heal followers by taking them back to the base and ensuring you have a functioning infirmary. They'll automatically receive treatment.
To heal a follower in State of Decay, make sure you have medical supplies in your inventory. Then, just approach your follower and press the appropriate button to give them the meds. Simple as that!
