First, close the game. Locate your old save file, usually in `Documents/My Games/Binding of Isaac Afterbirth`. Copy the `persistentgamedataX.dat` files and paste them into the same folder on the new machine or installation. Restart the game and boom, your saves should be there!
It's just copy-pasting `persistentgamedataX.dat` from the old to the new directory. Simple as that!
