Get an FPS config! Check out FPS packs like Chris' config or mastercomfig that optimize settings specifically for TF2.
Honestly, just upgrade your PC. Some old rigs just can't handle the awesomeness of TF2!
Install the game on an SSD if you haven't already. Faster drive = better performance.
Lower your graphic settings in the options menu. Trust me, TF2 still looks great even when you dial things down a bit!
