Super easy to spot! When a female dweller is pregnant, she'll start walking around with a baby bump. Plus, she won't be wearing her usual vault suit but a yellow one instead. Another dead giveaway is you can't assign her to any dangerous tasks or explorations outside the Vault. Just keep an eye on your dwellers after they've had a little 'private time' in the living quarters, and you'll notice the changes.
