As someone who’s done this, go to your Ubisoft Connect settings and look for Linked Accounts. Click on the Xbox icon and follow the prompts to log in.

Just head to the Ubisoft Connect website, log in, go to Account Information, and link your Xbox account there. Easy peasy!
