Just plant Mushroom Grass Seeds on mud. That's it. Well, patience too, lots of it.

Making a mushroom biome in Terraria is pretty simple but requires some patience. First, you gotta get yourself some Mushroom Grass Seeds. You can grab these from glowing mushrooms in an underground mushroom biome. Once you have the seeds, find a nice spot, preferably in the Underground layer because it'll spread faster. Now, just plant those seeds on some Mud Blocks and wait. The biome will start forming as the glowing mushroom grass spreads. If you're feeling fancy, add more mud blocks and plant more seeds to speed things up. Just remember, the bigger the area, the more likely it is to attract Truffle, the mushroom NPC, but only in hardmode!

Honestly, why bother making one manually? Just explore and you'll stumble upon one eventually. Then, use it to your heart's content!
