To make a Mushroom Biome in Terraria, you gotta start by gathering some Mushroom Grass Seeds from an existing Mushroom Biome. You can find these biomes usually underground. Once you have those seeds, find yourself a nice spot (preferably in your own created world so no one messes with your masterpiece) and start planting them on some Mud Blocks. Remember, these seeds need to be planted on Mud, not regular dirt, so make sure you got plenty of Mud Blocks ready. After planting, just give it some time, and voila, your very own Mushroom Biome will start to sprout. Just keep in mind, it needs to be fairly large to spawn the unique mobs and to truly be considered a biome!

All you need is some Mushroom Grass Seeds and Mud Blocks. Plant the seeds on the mud and wait. Easy as pie!

Honestly, why bother making a whole biome? Just find one and set up camp. Much easier.
