You can create an End portal using 12 end portal frames and 12 Eyes of Ender. Arrange the frames in a 3x3 square with corners empty, then place the Eyes of Ender in each frame.

To make a Nether portal, you need obsidian blocks and flint and steel. Create a vertical frame 4 blocks tall and 5 blocks wide with the obsidian, then light it with the flint and steel!
