From personal experience, it's soul sand T shape first, then wither skulls on top. Gear up before you do though... it's gonna get wild.

To summon a Wither in Minecraft PS3, you gotta place four blocks of soul sand in a T shape, then pop three wither skeleton skulls on top. Boom, Wither time!
