Honestly, just download a template. There's tons of them online. Edit it with whatever software you're comfy with, could be anything from Photoshop to MS Paint if you're brave enough. Print that bad boy on some nice cardstock, cut, fold, glue, and done. Easier than trying to figure it out from scratch.
