To increase the chances of having twins, make sure your Sims have the highest relationship level and use the ‘Super Romantic’ interaction before trying for a baby. You can also use the Pregnancy Event and complete all tasks for a higher chance!

I tried the 'Super Romantic' interaction, maxed out their relationship, and did the Pregnancy Event tasks. My Sims had twins! Good luck!

Honestly, it's mostly luck. But try having your Sims in a great mood and upping their relationship level. Worked for me once!

No way to guarantee twins. But try maxing out the romantic relationship and using a 3-star bed for better chances.
