To make your Sims pregnant in The Sims FreePlay, first, make sure they're married. Then, buy a cot from the Home Store, tap on it, and select the 'Add a Baby' option. Bam! Soon you'll have a new little Sim running around!

Get your Sims married, buy a crib, and then you can use the 'Add a Baby' option from the crib to start the magic. Babies incoming!

From personal experience, you need to marry your Sims first. After that, place a crib and choose 'Add a Baby'. Worked every time for me!
