If you're using Uplay, just go into your Uplay client settings, find the 'Install Locations' setting, and change it to the new drive. Then, move the game folder manually to the new location. Restart Uplay and it should recognize the updated location. Done and done!

For Steam users, first, go to the game鈥檚 properties, then 'Local Files' and select 'Move Install Folder.' Choose your new hard drive, and Steam will handle the rest! Easy peasy.

I've done this: Copy the whole game folder from its original location to the new drive. After that, open Uplay, go to 'Downloads', and change the 'Game install location' to the new folder. Uplay will verify files and you鈥檙e good to go!
