To open the mysterious door in Undertale, first you need to achieve a True Pacifist ending. After that, go back to the Mysterious Door in Snowdin and you'll find that it's open! Make sure you’ve befriended everyone!

Get the Dog Residue item and use it repeatedly until you fill up your inventory with Dog Residue and Dog Salad. Head to the door in Snowdin, and it’ll open up on its own!

You need to dodge all of Mettaton EX's attacks without getting hit. Yep, perfection is the key. Once you do that, head to the mysterious door in Snowdin to find it open (plus, you'll get the 'Good Time' achievement!).

I've done it myself! Beat the game with a True Pacifist run, then return to the door in Snowdin. You'll find it swings wide open, revealing a fun Easter egg!
