Go to the PlayStation Store on your console, scroll to the bottom of the sidebar, and select 'Redeem Codes.' Enter your 12-digit code and hit 'Continue.' Boom, you're done!

If you're on the PS4, open the PlayStation Store, scroll to the sidebar, select 'Redeem Codes,' enter your code, and voilà, your subscription is active!
