For me, turning off the PS4 completely and then rebooting while the controller is plugged in usually does the trick.

Honestly, buy a new controller. If it's not connecting, it might be dead.

If the USB method doesn’t work, try resetting the controller by pressing the tiny reset button on the back with a paper clip or pin. Then, connect it again with the USB cable.
