To record a clip on your PS4, just double tap the Share button on your controller. It'll automatically start recording your gameplay. You can stop by pressing the Share button once more and selecting 'Save Video Clip'.

From experience, just press the Share button, go to 'Save Video Clip', and boom, you've got your footage. Super easy!
