Contact Supercell support through the settings menu in-game. They can help you recover your old farm, but make sure to provide as much info as possible like your farm's name and level.
If you never linked your farm to any account and have a new device, you might be out of luck. Always connect to Facebook or Google Play in the future to save your progress.
I lost my farm once and got it back by just re-logging into Game Center on my iPhone. Magic, I tell you!
