To refund V-Bucks on Fortnite for PS4, go to Settings > Account and Privacy > Submit a Request. You'll see all eligible items for refund. Remember, refunds are limited to 3 lifetime refund tokens.
From personal experience, head to the Fortnite settings, then Account and Privacy, and choose 'Submit a Request' to refund items.
Just a heads up: You can't refund V-Bucks directly on PS4, only the items you've bought with them!
