You could just stop playing the game... but if you insist, see the 'Apps and Websites' section in settings.
Just head to the Facebook settings, then 'Apps and Websites', locate the game, and hit 'Remove'. Done and dusted!
Been there, done that! I went to settings, clicked 'Apps and Websites', and booted the game right out of there.
