Simply navigate to Facebook Settings -> Apps and Websites -> Active, find Sims FreePlay, and remove it. You're free!

I had to do this once. Tap the menu in Facebook, go to Settings & Privacy, find Apps and Websites, choose Sims FreePlay, and hit Remove. Felt like a boss after!
