Just hit Tab, right-click the player's name, choose 'Report Player', and follow the prompts. No biggie.

Click Tab to open the scoreboard, right-click the user, and select 'Report Player'. Valve will do the rest!

To report a player in Team Fortress 2, open the scoreboard (default key: Tab), right-click the offending player's name, and choose 'Report Player'. Easy peasy!
