In the game's menu, navigate to Options, select Controls, and just hit the triangle button. Boom! Back to default.

Heads up: you just need to go to Options -> Controls -> Press Triangle.

Go to Options > Controls, then press the triangle button to reset to default. Easy peasy!

Look for the 'Options' tab in the main menu, choose 'Controls,' and then press triangle to reset. I had to do this once after my dog decided he was a better gamer than me!
