Hold down the PlayStation and Share buttons simultaneously until the light bar starts flashing. This will factory reset your PS4 controller.

Use a USB cable to connect the controller to your PS4, then press the PlayStation button. This can help re-sync and reset it.

Once I had to do this, and holding the PlayStation button for 10 seconds while plugged into the console did the trick.
