Just cap it at 60fps, guys. The game wasn't designed for 144Hz, and trying to force it can cause more trouble than it's worth. Enjoy the game as it was meant to be played!

I managed to get Fallout 4 running smoothly at 144Hz by tweaking the .ini files as mentioned, but also made sure my GPU settings were optimized for Performance. It took a bit of fiddling, but the difference is night and day. Make sure to backup your files before you start tweaking!
