Simply go to a wardrobe in your safe house or a clothing store, put together your outfit, and then hit the option to 'save outfit.' Done and dusted.

Actually, why bother saving outfits? Just enjoy the game and wear whatever you pick up. Makes for a more interesting story, IMO.
