Turn off the PS4 completely. Hold the power button until you hear a second beep (about 7 seconds). Connect a controller with a USB cable and press the PS button to enter Safe Mode.

First, make sure the PS4 is off. Press and hold the power button. Release after the second beep. Plug in your controller via USB and press the PS button.
