I've done it! Get your smarts and craziness stats high, then become famous. You'll find the 'Start a Cult' option in the Relationships section.

First, make sure your character has high Smarts and Craziness. Get famous, then unlock the option to start a cult under the Relationships tab. Easy peasy, cult-leader-squeezy!
